Incidence of Myocardial Infarction Following Elective Total Hip Arthroplasty: A Population‐Based Study

Background:

The 2007 ACC/AHA Perioperative Guideline groups elective hip arthroplasty with all other orthopedic procedures in a risk category of <5% of myocardial infarction (MI) or death within 1 year. Very little is known specifically about the incidence of MI following elective total hip arthroplasty (THA).

Methods:

The study was a population‐based retrospective cohort of 692 elective total THAs in Olmsted County, Minnesota, between January 1, 1988, and December 31, 2002. Data were obtained through complete medical record review of all inpatient, outpatient, and nursing home care for at least 1 year following the incident surgery.

Results:

Mean age was 74.9 years, 63% were female, and 99% were white. The incidence of postoperative MI following THA was 4.5% (95% CI: 2.9, 6) at 7 days and 4.8% (95% CI: 3.2, 6.3), 5.1% (95% CI: 34, 6.7), and 6% (95% CI: 4.2, 7.8) at 1 month, 6 months, and 1 year, respectively. Deaths were censored. More than 75% of postoperative MIs occurred during surgical hospitalization. The incidence of MI or death at 1 year was 8.2%.

Conclusions:

The 1‐year incidence of MI following elective hip arthroplasty was 6%, with approximately 75% of those occurring within the first 7 postoperative days. The postoperative risk of MI or death (8.2%) in THA patients was slightly higher than the published ACC/AHA risk categorization (5%). More research is needed to determine the most appropriate properties of risk stratification for different types of orthopedic surgery.
